The handles where the wheelchair is pushed by the nurse looks to be quite far up. Usually they are where the patient shoulders are or lower because pushers hands are more naturally positioned. Now it looks like the handles are on the patients head level (shoulder level for nurse). My suggestion would be to extend those bars coming from front wheels forming arm rests to be extended upwars to form pushing handles.

I might be looking this way too much from a ergonomical point of view and I don't know if the chair is modeled from a real reference.

The long back rest suggests it's been used to handle difficult patients like the ones in straitjacket and this generates mental hospital mood for this image (I think it's good thing). Think carefully what you want to tell with this chair as it's the focal point of the image. It's positioned right where the eye will be drawn by all perspective lines.

Overall the image has a good atmoshphere and the chair looks great.

Hey, did you try making it an old style wheelchair - big wheels in the front and the small ones at the back? It might help with the overall theme.

The chair looks uncomfortable to me, with those bars going through the cushions. Do (did) they really make chairs that way?

Unless he means from junkie squatters. It's not uncommon for abandoned buildings to be littered with used needles. It's a huge problem for police officers when searching for evidence after criminals hide/run through one, because they can accidently stab themselves with one and contract AIDS/HIV, even when using the latex gloves. Usually they'll get a canine unit for that and it makes a routine task take forever/more expensive.

You'd be surprised what gets left behind when a place closes. Obviously things like painkillers and copper pipe are going to be stolen very quickly, but it's reasonable to find lots of other things tucked into boxes or cabinets and eventually spilled by environment, animals, or looters. Things like medical supplies that were considered contaminated or written off and so left behind.

Paper should be everywhere. Unused letterheads, medical records, invoices, tons of documents that are too heavy or too numerous to dispose of properly (or people were too lazy) and considered generally valueless are going to be left to rot.
