It's all there on the Jam's FAQ page.

The lemon medal is the main 72hr medal. Just make something TF2-related within 72 hours, and if you clear the bar of "put-more-than-a-sneeze-worth-of-effort-into-your-submission", you'll get a medal when they get distributed in a month or two.

If you donate some money, you get the heart medal as well.
