Jump Pad Prefab

I would like to see a prefab for the jump pads used in the pass time gamemode, I have seen them in a few custom maps, but I have no clue how to use the entity (s) invovled, so maybe a tutorial or both of these?

I just finished a prefab. The vertical one requires a bit of tweaking, but the horizontal one is pretty good unless I messed something up.
