Random map crashes

The last couple of weeks I've been trying to playtest a map I built. On my computer it runs fine with any number of bots. By myself, I am unable to crash the map on the server. However, with players the map will cause everyone to simultaneously crash.

All users will have their clients freeze, and any sound playing at the time will loop indefinitely until half life is manually closed. The server will act as if they are still logged on, their avatars stuck dumbly where the player left them at the time of the crash.

In some instances error messages will be generated, although no two have been the same so far. Typically they are something like "the exception unknown software exception.." or the "instruction at ... referenced memory at... the memory could not be read"

Anyway, I cannot find any obvious cause to the error - any thoughts yall might have would be much appreciated. The latest version of the map can be downloaded here.

- it possibly manifests itself upon a player's death
- there are cameras on the level
- it has yet to crash before the first 2 points are captured (could a forward spawn be involved?)

this popped out at me...not entirely sure what it means...it shows up right before the light bounces...which you have an unusual amount of

As far as movement on the map, all I have is about 6 doors.

I have a theory that the error occurs upon a player's death - which causes a crash on their side and somehow this crashes everyone's client while leaving the server intact. Unfortunately, I can't prove this.

Hmm thats odd, the same problem happens to me. Once my ragdoll hits the ground in diffrent areas, it just freezes me. Also if i shoot a sticky bomb at the a certain point it crashes as well. Its very odd, iv done some tests and found out deleting the displacment i was shooting at with my stickys fixed the problem, but i still crash when my ragdoll touchs the ground. D:

I dont know why, i took EXTRA care when making these displacements. IDK why this is happening
