You put the .bsp file in your steamapps/USERNAME/team fortress 2/tf/maps/ folder.

In the main TF2 menu hit create server, and select the map from the dropdown list.

Alternitivly,

open the console and type "map koth_lolcano_b2" without the quotes.

Also, there have been a couple comments about team color. Do not change the lighting! It certainly is harder to distinguish teammates from enemies, but to me this adds another subtle layer of strategy and overall interest. I wouldn't want the effect on every map, but it's a fun challenge to have enemies "sneaking" up on me when I can't tell the difference.
