So when there is the "name" field do I write there my own name or do i write there " If I win, my prize shall be irrevocably named R4ge*'s Über domintion hat, and I am fine with that." ( not that i would name it like that). So tell me where i must write that text. plz 8)?

I suspect the "If I win..." bit was tounge-in-cheek. I just put Firstname "Nickname" Lastname.
