What does it mean to be 'off the grid'?

I've seen this posted in many places, however I cannot seem to figure it out for the life of me. Is this in regards to verticies not lining up with the grid somehow, or is it another meaning?

It can mean verticies being at decimal values (off the grid entirely, not aligned to a 1-unit grid), or it could refer to sloppy work in general where everything is done without gridsnap or on a tiny grid setting that results in things unslightly and misaligned even though they are all on integer values.
