cp_swanlake - non-symmetrical 3-point koth map

Non-symmetrical 3-point koth map: The more you're winning, more spread out your team must be to keep your lead.

There is a 30-second set-up time, allowing engineers to build dispensers, teleporters and sentries.

Depending on feedback, I might add rotating spawns & 2 mid spawns to prevent the map from feeling too linear.
