Do you have multiple sources of water on the map that aren't at the same height level? I believe this causes problems like that.

Nope, it's a Source engine limitation. You can't have either real-time reflective water or water that refracts on two different heights if they're visible at the same time.

Specifically, they can't ever both be in PVS. Try freezing the PVS from the console and checking if there is other water visible.

Wait, freeze the PVS? What's the command for that? I've never even heard of such a thing.

No guarantee that it will fix it. I had just 1 surface in my map and that one already messed up in similar ways: http://tf2maps.net/threads/broken-water-not-a-leak.26524/

Also, i dont know if lockpvs also does take areaportals into it, but if it does and you use them you should disable the areaportals aswel. But it can at least identify if the visleaf border is relevant and which area of the map could be the problem, and that problem might not have to be water.

if I understand this right: https://developer.valvesoftware.com/wiki/PVS
you can use r_novis 1 as well for that

r_novis 1 acts as if vvis never has been used, it renders the whole map.
