Was wondering if anyone made one and I didn't see it. I'm trying to add him in my achievement map and was wanting to save some time if someone already made it. If not does anyone happen to know what the entity name is for him?

The entity name is "merasmus".
I have maps with HH and monoculus build in... You can do it with a plugin but you need the portals and everything built into the map. The monoculus spawner at alliedmodders doesn't support the portals.
