I went through and deleted all the visclusters, added about 20 areaportals, and 1 occluder. I am getting around 300 fps, see if it is any better for you

This is just a guess; have you tried changing "airblast disabled" ... "heal on kill" to "airblast disabled" 1 ... "heal on kill" 5000 // or some amount

I checked L4D2, this is how they do it: Just a bunch of T-shaped cards as a model. Here I lazily skinned it over if this is sufficient:

I didn't think people would still be wanting to download this. I've updated the file to include 36 more stairs, proper visgrouping, and the option to use blockbullets instead of playerclip. EDIT: Sorry, had some strange issues with texture alignment. Should be all fixed now
