So, do you hold space and use the left/right mouse buttons to control the camera, or do you press z and move the camera with WASD and the mouse?

lol nice job missing option out :p!!

I like most others use WASD and the arrow keys, although if i need to move reeeaaly far I use the scroll wheel a bit

Mouselook (Z toggle) and WASD, occasionally I use scroll wheel for quick movements. I don't like having to move my right hand from mouse to the arrow keys.

I use Z to get me in free look mode and then a combination of WASD and the mouse w/ scroll. Why anybody would use arrow keys instead of your mouse is beyond lunacy in my book. So much easier to point your camera with mouse and then use WASD for your maneuvering. I mainly use scroll wheel in top view to zoom in for vertex work.

I use WASD, Spacebar, and the mouse wheel.

Using the spacebar and mouse-wheel feels more natural from programs like 3DSM and Photoshop.

The keys change though, but in PS it's just the arrow keys which are almost the same exact thing anyways - so it's all goooood.
