build one side up to mid. select all. copy. paste. rotate 180. line up with grid.

Make a massive skip brush under the map that is centered exactly at the middle of the map. Select what you want mirrored + the skip brush, copy, paste special and rotate 180°.
Repeat for everything added.

I'm almost at the point of doing this myself, what about after? I'm concerned with doors and spawn areas - these entities are named "red_x" - any way to change these names or at least list the named entities on the map?

Edit > Replace. Then you can replace the contents of everything or anything you have selected.

select all the entities > right click > paste special > check the box near the bottom "make names unique"
this will affect output the same and save you a lot of work

Will this retain references? That is, if entity1 gives entity2 as its parent, will that reference be retained? That is, if the system changes entity2 's name to entity2changed the parent of the entity1changed will refer to entity2changed.

Numerical transforms are a lifesaver.
FYI, Hammer as an tickbox option somewhere to restrict all rotations (done by dragging the handles in a 2d view using the mouse) to 15-degree increments unless you hold down ALT. Very handy.

yes, so long as all entities mentioned are copied

One thing that irks me about numerical transform is how it rotates objects around the center of their bounding box as opposed to their origin. Would be a nice option to have.

Don't forget to turn off vertices and model wireframes, lest your efforts be dampened by lag. I once spent 3 days flipping a map because it was slightly off.
